ℹ️ Formulation is an account, shared by a therapist and person, of the personal meaning and origins of a person's difficulties. This is viewed in the context of multiple factors, including relationships, social circumstances and life events and will indicate the most helpful way forward.
It is based on the PRSB Personalised Care and Support Plan Standard.
Formulation record entry
To create the formulation record, you will need to give it a name as well as providing a date, time and location of when and where the formulation was performed.
You can then provide details of the formulation including subheadings such as the 5 Ps (Presentation, predisposing factors, precipitating factors, perpetuating factors, protective factors), the 5 Ws (Who, what, when, where, why) and the SBAR (Situation-background-assessment-recommendation) model.
Finally, enter the details of the performing professional and the person completing record as well as the date the formulation was completed.
