Preferences, wishes, and beliefs regarding future care

ℹ️ An advance statement is a written statement that sets down your preferences, wishes, beliefs and values regarding your future care. The aim is to provide a guide to anyone who might have to make decisions in your best interest if you have lost the ability to make or communicate decisions.

It is based on the PRSB Personalised Care and Support Plan Standard.

Does the person have an advance statement?

If the person has an advance statement then click "Yes", which will open up the rest of the form. If they do not have one, you can select "No". You will then need to leave the date this decision was made.

Advance statement details

Please detail any written requests and preferences made by a person with capacity conveying their wishes, beliefs and values for their future care should they lose capacity. An advance statement can cover any aspect of your future health or social care.

This could include:

  • Where you would like to be cared for e.g, at home, in a hospital, a nursing home, a hospice

  • How you like to do things e.g. if you prefer a shower over a bath, or sleep with the light on

  • Practical issues e.g. who will look after your dog if you become ill

Once you have recorded the advance statement, you can log the date and time the advance statement was made, the location it was made as well as the location it is held.

You also have fields for the performing professional and the person completing the record.
