ℹ️ This is a record of the things that a person feels it is important to communicate about their needs, strengths, values, concerns and preferences to others providing support and care.

It is based on the PRSB About Me Standard.

What is the about me assessment?

This is a place to record the most important details that a person wants to share with professionals in health and social care. This can include:

  • Communication preferences

  • Likes and dislikes for personal comfort

  • How they prefer to take medication

The assessment is designed to be from the perspective of the person so all sections are framed as a statement.

Filling in the assessment

What is most important to me?

Here, you can document what is most important to the person including emergency information, values, spirituality/religion and meaningful activities

People who are important to me

This is where you can log the important people in this person's life. This can include family, friends, carers etc. You can include information about how the person wants these people involved in their life and who should/should not be contacted in relation to their care.

How I communicate and how to communicate with me

A description of how the person communicates, including any communication aids they use. Communication in different settings should be included, such as how they raise a concern, how they communicate when distressed and how they like to receive information.

My wellness

Details of what the person is able to do on a typical day through to a day where they are really unwell. You can log information about what might cause the person to be come unwell, signs that they are becoming unwell and past health issues.

Please do and please don't

A description of things that the person wants someone supporting them to do/not do. E.g. talk to me and not my carer.

How and when to support me

A description of when the person wants someone caring for them to support them. Include both emergency situations (e.g. taking blood) as well as day to day (e.g. personal care routines/moving and transitioning).

Also worth knowing about me

This is where the person can provide other information that they want someone supporting them to know. It can include a short history of their life as well as a profile of their current life.

Final sections

The final sections allow you to record the date completed, who supported them to write this statement (where relevant) and attach any multimedia files that are relevant to the assessment.
