Can CareLineLive provide a template for the consent form from the service user?

We do not have a recommended template. However, we refer you to guidance from NHS Digital here: GP Connect Privacy Notice.

Please review the section under the heading End user organisation privacy notice statement.

The copy for the consent form does not need to be complicated and we also advise that you revise your general Privacy Policy and Transparency Notice to include your usage of GP Connect so that for new service users, you will have consent 'automatically' from when they sign up to your services and accept your general Privacy Policy.
